West Muskingum Student Enrollment
Welcome to West Muskingum Schools! We appreciate your interest in our district.
The district has partnered with FinalForms, an online forms and data management service that replaces paper forms. FinalForms allows you to complete and sign K-12 student enrollment, back-to-school and athletic participation forms for your new or current student. We encourage you to review the Playbook that applies to your new or current student.
You can review and update your data at any time during the school year. You will be required to sign your online forms annually, and after any update you may make, such as a change of address or phone number. Registration must be completed by the parent or legal guardian.
For new students, the parent/guardian will need to provide the following documentation by either uploading copies in Final Forms, or bringing them to the office so copies can be made :
- Copy of birth certificate
- Immunization records
- Proof of residency (mortgage statement/lease agreement, utility bill, cable bill, etc.)
- Residency Affidavit (for families living in shared housing)
- Parent/Legal Guardian photo ID
- Divorce/Custody papers (if applicable)
- Current IEP/ETR/504 Plan (if applicable)

Open Enrollment
Families living outside of the West Muskingum School District interested in enrolling their student must create a new account under Final Forms and complete an Open Enrollment application. Approved and denied applicants will be notified by their child's building either by phone or email.
Kindergarten Registration
Children must be five years old by July 31 of the year kindergarten begins to enroll at West Muskingum Elementary.
(Children who will not be five on or before July 31, can be evaluated for possible early entrance to kindergarten. Evaluations must be scheduled through the Licking Regional Educational Service Center by calling (740) 349-6084.)
Preschool Enrollment
West Muskingum Schools house two preschools, which are operated by the Muskingum Valley Educational Service Center. Children who are age three and four as of August 1 are eligible to apply for enrollment.
Classes are held two full days per week, based on the the district/building calendar. Flexibility is available to allow families to set part-time schedules upon request. An income-based sliding fee scale is utilized to determine tuition costs. Transportation is usually available through the school district.
