Welcome to West Muskingum Schools!
The district has partnered with FinalForms, an online forms and data management service that replaces paper forms. FinalForms allows you to complete and sign K-12 student enrollment, back-to-school and athletic participation forms for your new or current student. We encourage you to review the Parent Playbook that applies to your new or current student.
You can review and update your data at any time during the school year. You will be required to sign your online forms annually, as well as after any update you make, such as a change of address or phone number. Registration must be completed by the parent or legal guardian. Registration opens March 1 prior to each school year. Be sure to select the correct school year when registering your child.
For new students, the parent/guardian will need to provide the following documentation by either uploading copies in Final Forms, or bringing them to the school office so copies can be made :
- Parent/Legal Guardian photo ID
- Copy of child's birth certificate
- Immunization records
- Proof of residency (mortgage statement/lease agreement, utility bill, cable bill, etc.)
- Residency Affidavit (for families living in shared housing)
- Custody documents, if applicable (shared parenting agreement, divorce decree, magistrate's order, court appointment, guardian, grandparent power of attorney, etc.). All court documents must have a court stamp and judge's signature. Notarized documents will not be accepted, per Ohio law.
- Current IEP/ETR/504 Plan (if applicable)

Kindergarten Registration
Effective for the 2026-27 school year, children who will be five (5) years old on or before the first day of school, (August 26), are eligible to register for kindergarten in accordance with Ohio Revised Code 3321.01. Be sure to select the correct school year when registering your child. Information regarding screenings and events will be released by the elementary.
Children who will not be five on or before the first day of school, can be evaluated for possible early entrance to kindergarten. Evaluations must be scheduled through the Muskingum Valley Educational Service Center by calling 740-452-4518.
Open Enrollment
Families living outside of the West Muskingum School District interested in enrolling their student must create a Final Forms account and complete an Open Enrollment application. Approved and denied applicants will be notified by their child's building either by phone or email.
In accordance with the Ohio Department of Education and Workforce West Muskingum Schools will transport open enrolled students. Open enrolled students will only be permitted to board buses at an established bus stop within the West Muskingum School District. Once a bus stop has been established, that will be the bus they ride throughout the school year. Anyone with questions can contact the Transportation Office at 455-4291.
Preschool Enrollment
Two preschools classes are located on the West Muskingum campus and are operated by the Muskingum Valley Educational Service Center. Children who are age three and four as of August 1 are eligible to apply for enrollment.
Classes are held two full days per week, based on the the district/building calendar. Flexibility is available to allow families to set part-time schedules upon request. An income-based sliding fee scale is utilized to determine tuition costs. Transportation is usually available through the school district.

Homeschooling
West Muskingum families homeschooling their student must submit their letter of intent by dropping it off at the administration center, M-F from 8 a.m. to 4 p.m., OR by mailing it to the attention of the superintendent, West Muskingum Schools, 4880 West Pike, Zanesville, OH 43701.
